  • Healthcare.Gov
    The "Go-To" website for information about the Affordable Care Act. This is the official government website and all current information, updates, and news can be found here. There is also a place to supply your email address and have news and updates about the Affordable Care Act sent to you directly.
     
  • Cuidado De Salud
    The Spanish language version of Healthcare.gov
    Documents and/or information needed if you want to enroll . . .

    Social Security Numbers

    Dates of Birth

    Permanent Resident Card or  other legal documents

    Employer name, address, and phone

    2012 income tax returns

    Check stubs for one month

    Other income information such as award letters, etc.

    Health insurance cards- if any

    E-mail user name and password.
