      How to get Tax Forms

The Internal Revenue Service made drastic cuts in providing

paper tax forms to SAPL. These service reductions were caused

      by Federal tax cuts.

      Here are some options:

      1) If you wish to order forms or instruction booklets, please call the Internal Revenue Service at 1-800-829-3676.

      2) If you wish to order tax forms to be mailed to your home, please visit http://www.irs.gov/Forms-&-Pubs/Order-Products.

      3) You may visit a VITA site where you will receive assistance in filing your taxes electronically.  Visit VITA at http://www.vitasa.org to find a VITA site near you.

      4) You may sign up for an Internet computer and visit http://www.irs.gov/Forms-&-Pubs to download and print tax forms.
