Where is the application?

The Volunteer Application form can be found below. It also lives on the Teen Volunteer Opportunities guide, and on the Organizational Health Unit Libguide under Volunteer Forms.

Where do I send it?

The completed Teen Volunteer Application can be sent in bin mail to the Volunteer Coordinator:

 Veronica Anderson; CEN- Organizational  Health/ Volunteer Services.

  • For In-person Volunteers: Scan the first page of the application, not only to keep teen's emergency contact info on file at the location the teen is volunteering at, but also so you can refer back to their availability and when the teen needs their volunteer hours.
  • For Off-site Volunteers: Please scan the first page of the application and email to the Off-site Teen Supervisor (currently This helps us out, because after the paper application is sent for the background check, it can be nearly impossible to get back.
  • Teens are welcome to simultaneously volunteer in-person and off-site!
  • Note: minors are not required to provide ID and their background check is free (unlike volunteers ages 18+)


When can the teen start?

Let the teen know that it may take a while (two weeks) for their application and background check to get processed.

  • When the background check is cleared, you, as volunteer liaison at the branch, will call the teen to schedule their orientation. (For virtual volunteers, the Off-site supervisor will lead the teen's orientation.)

If you need your volunteer for an event before their application has finished processing, have the teen and their adult complete the One-Day Volunteer Form.
